Now for your question - I would go back into home care full time until you can find a case management position if that is what you want. 12 yrs is not much when you have the knowledge to advance which you do have. There are a lot of self employed that have closed up shop and went back into the workforce. If an employer asks why the gap, tell them the truth and let them decide your fate. Just be honest with them and all will work out.

1) I don't know exactly. I teach medical front office. At some schools it is called Administrative Medical Assisting and at others Medical Administrative Assisting, even though it is basically the same program. I would compare the classes required and ask the schools what the differences are in their program and others. It may essentially be the same thing.

2) It is better to go to a university known for this career. There are employers who will not hire graduates of certain programs. So it won't be equal in pay if they won't hire you. Check with employers in your area to see which schools they prefer. And also ask which program they prefer, if either.
