Well as i can understand you want to write a letter to the management narrating your experience while working with the charitable trust hospital and not your working experience in a CV.

Please write your experiences while on the job in simple words, irrespective if it was good or bad, with regard to infrastructure (facilities during day time, emergency, diagnostic equipments, lacking facilities, behaviour of professional colleagues, staff and volunteers at help, with patients and every one including working team, availability of medicinal support, cleanliness and hygiene, food for patients and many more according to the environment of the workplace. Your pleasant experiences with regard to doctor - patient relations, ground for learning and how it could be upgraded, how will it help you in future etc.

I am thinking you might need to grt creative. Tell them what you are applying for specific position. what qualities you have to offer relevant to the position and specific to the company.(do some research} Let them know you are looking forward to their guidance and learning from their experience

I would send it in letter form, rather than email, email is a little informal for the process.
Tell them how much you would like to work there, and how you will be an asset to the team
Let them know you are hard working and reliable, with an open personality - they will appreciate you being approachable, as you will have to deal with customers - tell them you listen, when instructed and pick up new skills quickly, allowing you to work methodically, under your own initiative, once shown your tasks.
They will want to know that you are going to be compus mentis, and not some half asleep stereo type of a teenager, so you will want to relay the fact that, you work well in a team and that you will be able to do as your told and get on with colleagues.
Although I say to send it in a letter (it's more official, and if were the person reviewing the option for you, I would not even consider an email) they will need to know you are proficient with a computer, as they not only do appointments on the computer, but details of patients are kept on there, and visual programmes, so tell them how you feel at ease on the computer - maybe even tell them, that you thought of emailing them, but you considered a formal letter to be more respectful, they would get the message, you can use email, whilst appreciating that you gave respect a bit of thought.
Try just to think of everything you know is required in an opticians, then highlight your skills that match, they will obviously look to these more than irrelevant ones.
