Probably start in lower sixth. You can start as soon as possible if you want; start with the basics, so get yourself into the caring profession. Volunteer at nursing homes, hospices, etc.

When you're in lower sixth, you can move onto more specialised professions and spend two or three weeks off college assisting in infirmaries, clinics or maybe dentists if you want a backup plan.

You don't have to do the voluntary work, though. You can show you are caring through specialised professions too, I suppose. Make sure you do work experience in a suitable time of year, e.g. right now when exam pressure is beginning to happen is a bad idea.
